How protected health information about you may be used and disclosed, and how you can get access to it.
This Notice describes how medical information about you may be used and disclosed, and how you can get access to that information. Please review it carefully. It applies to protected health information ("PHI") created or received in the course of the care arranged through 20Science.
Treatment: your PHI is shared with the licensed clinicians who evaluate you and with the pharmacies that dispense your medication, so that you can be treated. Payment: PHI may be used to bill and collect for services and medication. Health care operations: PHI may be used for quality assessment, clinician review, and administration of the service. These uses do not require your separate authorisation.
PHI may be disclosed without your authorisation where the law requires or permits it: to public health authorities, in response to a valid subpoena or court order, to health oversight agencies, to avert a serious and imminent threat to health or safety, for workers' compensation where applicable, and to coroners or medical examiners. Any other use or disclosure requires your written authorisation, which you may revoke at any time except to the extent it has already been acted upon.
Most uses of psychotherapy notes, any use of PHI for marketing, and any sale of PHI require your explicit written authorisation. We do not sell PHI.
You have the right to inspect and obtain a copy of your health records, usually within 30 days; to request a correction if you believe something is inaccurate; to request restrictions on certain uses and disclosures; to request confidential communication by an alternative means or at an alternative address; to receive an accounting of certain disclosures; to be notified in the event of a breach affecting your unsecured PHI; and to obtain a paper copy of this Notice on request.
You may complain to us at support@20science.com, or to the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for making a complaint. Filing a complaint costs you nothing and does not affect your care.
